Endolift vs Thermage
Optical-Fibre Laser vs Monopolar Radiofrequency
Endolift vs Thermage: Which Is More Effective?
Endolift and Thermage are two different energy-based treatments that may be considered for facial, jawline, double-chin and neck laxity. Endolift is a minimally invasive laser procedure performed beneath the skin with a thin optical fibre. Thermage is a non-invasive monopolar radiofrequency treatment applied from the skin surface.
Endolift may be considered when more targeted subdermal contraction, jawline contouring and treatment of suitable small fat deposits are required. Thermage may be more appropriate when the goal is general skin tightening without needles, incisions or optical-fibre entry. The correct choice depends on individual anatomy rather than the popularity of either treatment.

A minimally invasive treatment that delivers laser energy to selected tissue planes beneath the skin through a thin optical fibre.
A non-invasive skin-tightening technology that delivers monopolar radiofrequency energy from the skin surface.
Endolift targets tissue directly from beneath the skin; Thermage delivers energy through a surface applicator.
Both aim to create controlled heating and a gradual collagen-remodelling response.
The Essential Answer: Endolift or Thermage?
Endolift may be preferred when the jawline, jowls, double chin or lower face require more targeted subdermal treatment. Thermage may be considered when the patient wants mild or moderate general skin tightening without needles, incisions or optical-fibre entry.
The fact that Endolift is minimally invasive does not mean that it will provide a better result for every patient. Likewise, the fact that Thermage is non-invasive does not mean that it will be sufficient for a pronounced subdermal contour problem.
Treatment selection should consider fat volume, skin thickness, laxity, facial volume, jawline structure, previous procedures and recovery expectations.
What Is Endolift?
Endolift is a minimally invasive laser procedure in which a very thin optical fibre is guided through pre-planned tissue layers beneath the skin.
The 1470 nm laser energy is intended to create controlled heating within connective tissue, supporting early tissue contraction and gradual collagen remodelling during the following months.
In suitable patients, limited contouring of small localised fat deposits may also be included. Endolift is not liposuction, and treatment of fat is neither required nor appropriate in every patient.
For more information, see the Endolift treatment page, how Endolift works and who is suitable for Endolift.
What Is Thermage?
Thermage is a non-invasive skin-tightening system that delivers monopolar radiofrequency energy through a specialised applicator placed on the skin surface.
During treatment, controlled radiofrequency energy is delivered to deeper tissue while cooling is used to help protect the skin surface. The thermal effect may cause controlled contraction of existing collagen and support new collagen formation over time.
Thermage FLX is a current-generation system that may be used across the face, eye area and selected body regions. Treatment is commonly planned as one main session, with results developing gradually.
Thermage does not use a fibre, cannula or microneedles beneath the skin. This distinguishes it from both Endolift and microneedling radiofrequency procedures.
Main Differences Between Endolift and Thermage
| Comparison | Endolift | Thermage |
|---|---|---|
| Energy type | 1470 nm laser energy | Monopolar radiofrequency energy |
| Delivery method | A thin optical fibre is guided through a selected tissue plane beneath the skin | Energy is delivered through a treatment tip placed on the skin surface |
| Invasiveness | Minimally invasive | Non-invasive |
| Primary purpose | Targeted tissue contraction, contouring and collagen support | General tightening, elasticity and collagen support |
| Approach to fat | Limited contouring may be considered in suitable small areas | Fat reduction is not its principal purpose |
| Session plan | Usually one main session | Usually one main session; repeat treatment depends on individual need |
| Recovery | Swelling, bruising, tenderness and temporary firmness may occur | Usually little social recovery; temporary redness or tenderness may occur |
| Result character | More targeted contour and subdermal support | More general, gradual and natural tightening |
| Common treatment areas | Jawline, jowls, double chin, lower face, neck and selected body areas | Face, eye area, jawline and selected body areas |
Who May Be More Suitable for Endolift?
- Patients seeking targeted lower-face and jawline contouring
- People with mild or moderate subdermal tissue laxity
- Patients concerned about early jowls or jawline blurring
- People with mild double-chin fat combined with laxity
- Patients who prefer treatment planned around one main session
- People who accept a minimally invasive optical-fibre procedure
- Patients who require more targeted treatment than a surface device may provide
- People considered anatomically suitable after medical assessment
Endolift may be particularly relevant when the concern is related to subdermal support, small fat deposits and contour rather than the skin surface alone.
Who May Be More Suitable for Thermage?
- People who do not want needles, incisions or optical-fibre entry
- Patients with mild or moderate general skin laxity
- People seeking natural tightening across the face, eye area or jawline
- Patients without pronounced localised fat excess
- People who do not want visible social recovery
- Patients seeking a one-session non-invasive technology
- People with thinner faces where preservation of facial volume is important
- Patients considered suitable for monopolar RF after medical assessment
Thermage may be more appropriate for people seeking gradual, restrained tightening rather than a pronounced contouring procedure.
Endolift or Thermage for the Jawline?
When jawline blurring is related to subdermal laxity, early jowls or small localised fat deposits, Endolift may offer a more targeted approach.
When the jawline is generally preserved but the skin has mild laxity or reduced elasticity, Thermage may be sufficient.
Jawline definition is not determined by skin tightness alone. Chin projection, lower-jaw support, facial volume and neck anatomy also influence the result.
When structural support is limited, chin filler or jawline filler may also be considered.
Which Is Better for a Double Chin?
When mild double-chin fat and skin laxity occur together, Endolift may address tightening and limited contouring within the same plan.
When there is no meaningful fat excess and mild skin laxity is the main concern, Thermage may be considered. Thermage is not principally designed to remove double-chin fat.
With a thicker or more pronounced submental fat layer, neither Endolift nor Thermage may be sufficient. Surgical assessment for double-chin liposuction or another treatment may be required.
For more information, see Endolift vs double-chin liposuction.
Which Is More Effective for Neck Tightening?
Thermage may be considered when the neck has mild general laxity and the patient prefers a non-invasive procedure.
Endolift may be more suitable when targeted tissue contraction or contouring is required across the transition between the chin and neck.
The neck skin can be thin, so treatment depth and energy should be planned carefully. Unnecessary fat loss, prolonged firmness or contour irregularity should be avoided.
Neither method replaces surgical neck lifting when substantial excess skin or prominent platysma bands are present.
Which May Be Preferred Around the Eyes?
Thermage has specialised treatment tips designed for selected eye-area concerns, including mild laxity and fine wrinkling.
Endolift around the eyes requires highly selective patient assessment and advanced anatomical knowledge. Not every eye-area concern is suitable for Endolift.
Excess eyelid skin, under-eye bags, pigmentation and volume loss are different problems. One energy-based treatment cannot correct all of them equally.
The eye area should be assessed for skin quality, fluid retention, fat position and underlying anatomy before treatment is selected.
Which Provides More Tightening?
Because the optical fibre is guided through selected tissue planes beneath the skin, Endolift may create more direct and targeted contraction in specific areas.
Thermage applies controlled monopolar radiofrequency across a wider area from the skin surface, supporting general tightening.
The Endolift result may therefore be perceived more as targeted contouring and regional support, while Thermage may create a broader change in firmness and elasticity.
Both treatments may have limited capacity in advanced laxity, where a surgical face- or neck-lift assessment may be more appropriate.
Which Is More Painful?
During Endolift, patients may feel warmth, pressure, pulling or movement of the optical fibre beneath the skin despite the planned anaesthetic or comfort measures.
During Thermage, each energy pulse may produce a deep heating sensation. Current systems may use vibration and cooling to improve comfort, although sensitivity varies.
Post-treatment tenderness and swelling generally last longer after Endolift. Most Thermage patients can return to daily activities quickly.
Pain and comfort depend on the treatment area, settings, individual sensitivity and overall treatment extent.
How Do Recovery Times Compare?
After Endolift
- Swelling and oedema
- Bruising
- Tenderness to touch
- Temporary tightness or firmness
- Temporary numbness
- Temporary asymmetry related to swelling
- Social recovery depending on the treatment area
After Thermage
- Temporary redness
- Mild swelling
- Temporary tenderness
- Occasional short-term numbness
- Usually no bandages
- Generally rapid return to daily activities
- Gradual development of results
Thermage generally involves less social recovery than Endolift. For more information, see the Endolift recovery guide and Endolift aftercare guide.
When Do Results Appear?
| Period | Endolift | Thermage |
|---|---|---|
| First days | Early contraction may be visible, although swelling can mask the result | Temporary firmness or a tighter sensation may be noticed |
| First weeks | Contour becomes easier to assess as swelling settles | Gradual tightening and skin-quality changes may become noticeable |
| First months | Collagen remodelling and tissue tightening continue to develop | Results may become more visible through the developing collagen response |
The appearance during the first few days should not be considered the final result of either treatment. See when Endolift results appear.
How Long Do Results Last?
Neither Endolift nor Thermage stops the natural ageing process. Result duration is influenced by age, skin quality, sun exposure, smoking, weight change and lifestyle.
After Endolift, the need for future treatment is assessed according to tissue response and new ageing changes.
Thermage is commonly planned as one main session, although a future maintenance or repeat session may be considered according to individual need.
For Endolift, see how long Endolift results last.
Side Effects and Risks
Possible Endolift Risks
- Swelling, bruising and tenderness
- Temporary or prolonged numbness
- Burns or pigmentation changes
- Infection
- Asymmetry or contour irregularity
- Unwanted fat loss or hollowing
- Firmness or nodules
- An unsatisfactory result
Possible Thermage Risks
- Temporary redness and swelling
- Pain or intense heat during treatment
- Temporary tenderness or numbness
- Burns or blistering
- Surface irregularity
- Rare unwanted fat loss
- Asymmetry

Device knowledge, correct energy settings, anatomical planning and patient selection are important for both treatments. See Endolift side effects and is Endolift safe?
Who May Not Be Suitable?
- People with active infection or an open wound in the treatment area
- Patients with uncontrolled systemic disease
- People taking medication that affects healing or bleeding
- Pregnant or breastfeeding patients
- Patients with advanced excess skin
- People expecting the result of surgical lifting
- Patients with a medical contraindication to energy-based procedures
- People with pacemakers or electronic implants whose RF suitability has not been confirmed
Previous filler, thread lift, HIFU, radiofrequency, Endolift, laser or surgical treatments should be disclosed during consultation.
Can Endolift and Thermage Be Combined?
In selected patients, Endolift and Thermage may be planned at different times for different tissue objectives. Unnecessary cumulative heating of the same area should be avoided.
For example, Endolift may be used for subdermal contour and connective-tissue support, while Thermage may be considered later for general tightening or maintenance.
Using both treatments does not automatically produce a better result. One appropriately selected treatment may be sufficient.
The interval should be determined according to the energy used, treatment extent, healing and individual skin characteristics.

The Efem Method: Identify the Source of the Concern First
Within the Efem Method, treatment is not selected according to device popularity or a pre-decided patient preference. The first step is to identify the tissue layer responsible for the laxity.
Endolift may be considered when subdermal contour, early jowls or small fat deposits are prominent. Thermage may be more appropriate when the priority is general skin laxity and a non-invasive treatment.
The objective is not to apply the greatest amount of energy, but to achieve an appropriate result while preserving natural facial volume.
How Should Costs Be Compared?
Endolift and Thermage should not be compared by session count alone. Although both are commonly planned as one main session, their technologies, treatment areas and procedure requirements are different.
Factors Affecting Endolift Cost
- Size of the treatment area
- Extent of the fibre pathways
- Degree of fat and laxity
- Treatment of multiple areas
- Procedure time and technical difficulty
- Follow-up plan
Factors Affecting Thermage Cost
- The Thermage system and treatment tip
- The number of treatment pulses
- Face, eye or body treatment
- Size of the treatment area
- Personalised energy plan
- Future maintenance or repeat treatment
The least expensive session is not necessarily the most appropriate treatment. See the Endolift cost guide and factors affecting Endolift cost.
Planning Treatment from Abroad
International patients travelling to Istanbul may request an initial review by sending clear front, profile and 45-degree photographs.
Because Thermage generally does not involve significant social recovery, it may be easier to include within a short Istanbul visit.
Swelling or bruising may occur after Endolift, so flights, work commitments and social events should be planned accordingly.
A photographic review does not replace an in-person consultation. Skin thickness, fat distribution, laxity and facial volume are assessed more accurately in person.

Before treatment, the capacity, session process, recovery, possible risks and realistic result level of Endolift and Thermage are discussed clearly.
The objective is not to recommend the same technology to every patient, but to create a natural and balanced plan that respects individual facial anatomy.

What Is a Realistic Result?
In a suitable patient, Endolift may support a clearer jawline, a more balanced lower-face transition and a firmer appearance.
Thermage may help the face appear tighter, more rested and better supported without changing its basic character.
Neither treatment produces the same correction as surgical lifting. Results depend on age, skin quality, laxity, fat volume and individual healing response.
Before-and-after photographs should be compared using the same lighting, angle, distance and facial position.

Statements such as “Endolift is automatically better because it is more invasive” or “Thermage is suitable for everyone because it requires little recovery” are inaccurate.
Endolift can reach selected tissue planes more directly. Thermage is a broader, surface-applied, non-invasive tightening approach.
The best result comes not from the strongest device, but from applying the appropriate method conservatively to the correct anatomical problem.

Is Endolift radiofrequency?
No. Endolift uses 1470 nm laser energy delivered through a thin optical fibre.
Which treatment is more invasive?
Endolift is minimally invasive because the optical fibre is guided beneath the skin. Thermage is non-invasive and applied from the skin surface.
Which is better for the jawline?
Endolift may be more suitable when early jowls, small fat deposits and subdermal laxity are present. Thermage may be considered for mild general laxity.
Which is better for a double chin?
Endolift may be more targeted when mild fat and laxity occur together. Thermage may be considered when mild laxity is present without meaningful fat excess.
Which is more suitable for the neck?
Thermage may suit mild general neck laxity. Endolift may be considered when targeted contouring is needed across the chin-to-neck transition.
Which is better around the eyes?
Thermage has specialised treatment tips for selected eye-area concerns. Endolift must be planned very selectively around the eyes.
Which has a shorter recovery?
Thermage generally involves less social recovery. Swelling and bruising may occur after Endolift.
Which treatment is more painful?
Both can produce a heating sensation. Endolift is minimally invasive, while Thermage pulses may create deep heat. Comfort varies between patients.
How many Endolift sessions are needed?
Endolift is usually planned as one main session. Any future treatment depends on tissue response and ongoing ageing.
How many Thermage sessions are needed?
Thermage is commonly performed as one main session. Maintenance or repeat treatment may be considered according to individual need.
Does Endolift reduce fat?
Limited contouring may be considered in suitable small localised areas. Endolift does not replace surgical liposuction.
Does Thermage melt fat?
The main purpose of Thermage is skin tightening through controlled radiofrequency heating, not targeted fat removal.
Can Endolift and Thermage be combined?
They may be planned at different times in selected patients. The treatment interval and total energy exposure should be determined medically.
Are Thermage and Ultherapy the same?
No. Thermage uses monopolar radiofrequency, while Ultherapy uses focused ultrasound energy.
Are Thermage and Morpheus8 the same?
No. Thermage is a surface-applied monopolar RF treatment. Morpheus8 delivers radiofrequency energy through microneedles.
Are Endolift and Thermage safe?
Both have possible risks. Safety depends on patient selection, technology, energy settings, anatomical planning and practitioner experience.
Which treatment costs more?
Cost depends on the treatment area, Endolift fibre plan, Thermage treatment tip, pulse count and individual treatment extent.
Which is closer to a surgical facelift?
Neither treatment replaces a surgical facelift. Endolift may be more targeted, but advanced excess skin and tissue descent may require surgical assessment.
