INFORMATION NOTICE ON THE PROCESSING OF PERSONAL DATA

Pursuant to the Turkish Personal Data Protection Law No. 6698 (“KVKK”), your personal data may be processed by Dr. Abdurrahman Efem (Efem Clinic), acting as the data controller, within the scope described below.

The data controller, Dr. Abdurrahman Efem, is based in Istanbul. The address is Levent Mahallesi, Nispetiye Cad. Krizantem Sok. No:12, Istanbul, Türkiye.

Dr. Abdurrahman Efem is registered with the Data Controllers’ Registry Information System (“VERBİS”).

Purpose of Processing Personal Data

Within the scope of the healthcare, medical and aesthetic services provided by Dr. Abdurrahman Efem, the following categories of personal data may be processed:

• identity information, including your name, surname and Turkish Republic identification number;

• contact information, including your telephone number and home or workplace address;

• health-related information, including your medical history where required for the provision of healthcare, medical or aesthetic services; and

• image and audio data where necessary to assess, document or understand the results of the services provided.

Your personal data will be processed within the personal data processing conditions and purposes set out in Articles 5 and 6 of KVKK.

Personal data processed within this scope will be retained for a period of 10 years.

To Whom Personal Data May Be Transferred and for What Purposes

The personal data listed above may be shared only:

• with authorised public institutions and authorities where disclosure is required by law; and

• with the Stellamedi (Softage Yazılım A.Ş.) record management system used by the Clinic, subject to applicable legal obligations relating to personal data protection and security.

Unless you request that your image data remain anonymous, such data may also be retained in Dr. Abdurrahman Efem’s institutional archives, provided that access is restricted to authorised members of the Clinic team.

Your image data may be processed and published on Dr. Abdurrahman Efem’s social media accounts only where you have provided your explicit consent. Such use may include the representation and communication of the relevant content to the public in accordance with the applicable provisions of the Turkish Law on Intellectual and Artistic Works.

Where explicit consent has been provided for this purpose, the relevant image data may be made publicly available through the agreed communication channels.

Method and Legal Basis for Collecting Personal Data

Your personal data is collected electronically and processed by non-automated means for the purposes described in this Notice.

The legal basis for such processing includes the necessity of processing personal data in connection with the performance of contractual obligations arising from the healthcare, medical and aesthetic services provided by Dr. Abdurrahman Efem, in accordance with the applicable provisions of KVKK.

Your Rights Under KVKK

As a data subject, you may exercise your rights under Article 11 of KVKK in relation to the processing of your personal data.

These rights include the right to:

• learn whether your personal data is being processed;

• request information regarding the processing of your personal data;

• learn the purpose of the processing and whether your personal data is being used in accordance with that purpose;

• learn the third parties to whom your personal data has been transferred;

• request the correction of incomplete or inaccurate personal data;

• request the erasure or destruction of personal data where the applicable legal conditions are met;

• request that correction, erasure or destruction operations be communicated to third parties to whom your personal data has been transferred;

• object to a result arising against you as a consequence of the analysis of your personal data exclusively through automated systems; and

• claim compensation where you suffer damage as a result of the unlawful processing of your personal data.

Requests concerning these rights may be submitted to Dr. Abdurrahman Efem, as the data controller, through the applicable communication channels.

Your request will be assessed according to its nature and concluded as soon as possible and, in any event, within 30 days.

Requests are generally processed free of charge. However, where responding to the request requires an additional cost, a fee may be charged in accordance with the tariff determined by the Turkish Personal Data Protection Board.
