INFORMATION NOTICE ON THE PROCESSING OF PERSONAL DATA

(For Online Appointment Requests)

Pursuant to the Turkish Personal Data Protection Law No. 6698 (“KVKK”), your personal data may be processed by Dr. Abdurrahman Efem (Efem Clinic), acting as the data controller, within the scope described below.

The data controller, Dr. Abdurrahman Efem, is based in Istanbul. The address is Levent Mahallesi, Nispetiye Cad. Krizantem Sok. No:12, Istanbul, Türkiye.

Dr. Abdurrahman Efem is registered with the Data Controllers’ Registry Information System (“VERBİS”).

Purpose of Processing Personal Data

Personal data submitted in connection with an online appointment request, consisting of your name and surname, telephone number and email address, may be processed for the purpose of managing appointment requests and carrying out processes related to the healthcare, medical and aesthetic services provided by Dr. Abdurrahman Efem.

Your personal data will be processed within the personal data processing conditions and purposes set out in Articles 5 and 6 of KVKK.

The personal data processed within this scope will be retained for a period of 10 years.

To Whom Personal Data May Be Transferred and for What Purposes

The personal data listed above may be shared only:

• with authorised public institutions and authorities where disclosure is required by law; and

• with the Stellamedi (Softage Yazılım A.Ş.) record management system used by the Clinic, subject to applicable personal data protection and security obligations.

Personal data will not be transferred for purposes incompatible with those described in this Notice.

Method and Legal Basis for Collecting Personal Data

Your personal data is collected electronically through the website when you submit an online appointment request.

The data is processed for the purposes described in this Notice and on the legal basis that processing is necessary for the establishment or performance of a contractual relationship and for the fulfilment of the related obligations, in accordance with the applicable provisions of KVKK.

Your Rights Under KVKK

As a data subject, you may exercise your rights under Article 11 of KVKK in relation to your personal data.

These rights include the right to:

• learn whether your personal data is being processed;

• request information regarding the processing of your personal data;

• learn the purpose for which your personal data is processed and whether it is being used in accordance with that purpose;

• learn the third parties to whom your personal data has been transferred, whether in Türkiye or abroad;

• request correction of incomplete or inaccurate personal data;

• request the erasure or destruction of personal data where the conditions provided by law are met;

• request that correction, erasure or destruction operations be communicated to third parties to whom your personal data has been transferred;

• object to a result arising against you as a consequence of the analysis of personal data exclusively through automated systems; and

• claim compensation where you suffer damage as a result of unlawful processing of your personal data.

Requests relating to these rights may be submitted to Dr. Abdurrahman Efem, as the data controller, through the applicable communication channels.

Requests will be assessed according to their nature and concluded as soon as possible and, in any event, within 30 days, in accordance with KVKK and applicable regulations.

Requests are generally processed free of charge. Where responding to a request requires additional costs, a fee may be charged in accordance with the tariff determined by the Turkish Personal Data Protection Board.
