What Is Radiesse? A Guide to Calcium Hydroxylapatite and Biostimulation

Radiesse is an injectable made by Merz from calcium hydroxylapatite (CaHA), a mineral that also occurs in bone and teeth. It gives immediate structural support and, over the following months, stimulates the body to produce its own collagen. This guide explains what it is, the forms it comes in, where it is used, what treatment involves and what to know about safety, with links to our more detailed articles on each question.

KEY POINTS

Radiesse in Five Points

  • What it is: CaHA microspheres of about 25–45 microns suspended in a gel carrier.
  • What it does: immediate support from the gel, then collagen formation around the microspheres as the gel is absorbed.
  • Where it is used: cheeks, jawline, folds around the mouth and the back of the hands; in diluted form, selected areas for skin quality.
  • How long: the FDA describes effects of roughly 18 months; the manufacturer calls the result long-lasting but not permanent.
  • Key safety point: unlike hyaluronic acid, it cannot be dissolved with an enzyme, so careful planning matters.

What Radiesse Is Made Of

Each syringe contains smooth calcium hydroxylapatite microspheres suspended in an aqueous gel carrier. Calcium hydroxylapatite is biocompatible and occurs naturally in the body; in Radiesse it is used as a scaffold rather than as a simple volume filler.

That is why Radiesse is usually described as a biostimulator rather than just a filler. It belongs to the same broad family as other collagen-stimulating injectables, which you can explore on our regenerative and biostimulatory treatments page.

The Different Forms of Radiesse

Form What it is Typical purpose
Radiesse The original CaHA product Structural support in the face and back of the hands
Radiesse (+) Radiesse pre-mixed with lidocaine, a local anaesthetic The same uses, with more comfort during injection
Diluted / hyperdiluted Radiesse Radiesse mixed with saline or lidocaine at 1:1 or higher ratios Skin quality and mild laxity rather than volume; an off-label technique based on expert consensus

HArmonyCa, which combines CaHA with hyaluronic acid, is a separate product. We compare the two in our article on Radiesse vs HArmonyCa.

How Radiesse Works

Radiesse acts in two stages. First, the gel carrier provides immediate support where it is placed. Then, as the gel is gradually absorbed, the microspheres remain and the body builds new collagen around them. Expert consensus describes new collagen from about four weeks, peaking around four months.

We explain the lifting effect in detail in how the Radiesse lifting effect works, and how this differs from hyaluronic acid filler over time in how Radiesse and HA filler behave in the skin.

Where Radiesse Is Used

The manufacturer's instructions list nasolabial folds, cheek augmentation, marionette lines, the jawline and volume loss on the back of the hands among its indications. At Efem Clinic, Radiesse is mainly planned for the mid face, lower face and jawline, and for skin-quality support in selected areas.

It is not used in the lips, and it is usually not our choice for the under-eye area. The manufacturer also lists the frown-line area and the nose as areas not intended for Radiesse. Our guide to who is a good candidate for Radiesse goes through who it suits and who it does not.

What Treatment and Recovery Involve

  • Assessment: anatomy, skin, previous treatments and goals are reviewed before any product is chosen.
  • Treatment: the product is placed with a fine needle or cannula at the planned depth. Radiesse (+) contains lidocaine to improve comfort.
  • Straight after: redness, swelling, tenderness and bruising usually settle within one to two days; facial swelling can take 7–10 days.
  • Over the following months: structural support is visible early, while collagen develops gradually.
  • Longer term: the FDA describes effects of roughly 18 months; area, technique and individual response all affect this.

Safety: What You Should Know

Radiesse has a long track record, but like every injectable it carries risks. The most common effects are temporary bruising, swelling and tenderness. Less often, lumps or nodules can form, particularly when the product is placed too superficially.

The most serious, though rare, risk with any filler is accidental injection into a blood vessel. An expert consensus on CaHA describes the warning signs: pain out of proportion to the treatment, skin that turns pale and then mottled, and, very rarely, changes in vision. Any of these during or after treatment needs urgent medical attention. Because Radiesse cannot be dissolved like hyaluronic acid, prevention depends on anatomy knowledge, careful technique and avoiding high-risk zones.

Radiesse is also visible on X-rays, CT scans and mammograms, so it is worth mentioning to any doctor who reviews your imaging.

Frequently Asked Questions

What is Radiesse made of?

Radiesse is made of calcium hydroxylapatite microspheres suspended in a gel carrier. Calcium hydroxylapatite is a biocompatible mineral that also occurs naturally in bone and teeth.

Is Radiesse a filler or a biostimulator?

Both. It gives immediate support like a filler, and it stimulates the body's own collagen over the following months, which is why it is described as a biostimulator.

How long does Radiesse last?

The FDA describes effects of roughly 18 months, and the manufacturer calls the result long-lasting but not permanent. Duration varies with the area, technique and individual response.

Does Radiesse hurt?

Most people feel pressure and some stinging. Radiesse (+) contains lidocaine, a local anaesthetic, which makes the injection more comfortable.

When will I see results?

Structural support is visible soon after treatment once swelling settles. Collagen develops over the following months, with expert consensus describing a peak around four months.

Can Radiesse be dissolved?

No. Unlike hyaluronic acid filler, Radiesse has no reversal enzyme. This is why careful candidacy assessment and technique matter.

What are the side effects of Radiesse?

The most common are temporary redness, swelling, tenderness and bruising. Less often, nodules can occur. Vascular complications are rare but serious and need urgent attention.

Is Radiesse the same as HArmonyCa?

No. HArmonyCa is a separate product that combines calcium hydroxylapatite with hyaluronic acid. Radiesse contains calcium hydroxylapatite in a gel carrier without hyaluronic acid.
